[ICU] Decline the Offer and Claim a Refund Online Application Form
（入学辞退および授業料・施設費返金オンライン申請）

In principle, once the Matriculation (Admission) Fee and Tuition/Facilities Fees have been paid, they will not be refunded. However, if one has to cancel matriculation for unavoidable circumstances, the Tuition/Facilities Fees shall be refunded upon request. The Matriculation (Admission) Fee is nonrefundable.
If you request a refund for Tuition/Facilities fees, please complete this form by the due date stated in the ENTRANCE PROCEDURES GUIDE. The application past the deadline is NOT acceptable. Please make sure to comply with the deadline.

原則として、一旦納入された入学金、授業料・施設費は返金されませんが、止むを得ない事情がある場合、授業料と施設費のみはお返しできる場合があります。入学金は返金できません。
授業料施設費の返金を希望する場合、入学手続き案内に記載の締切日までにこのフォームを提出してください。期日を過ぎた申請は受け付けませんので、ご注意ください。
