2025年度 ロームシアター京都自主事業の託児申込フォームです。
下記一覧より、ご来場公演を選択の上、個人情報のご記入へお進みください。

■お預かり対象年齢：3か月~就学前※小学生以上は要相談
■預かり料金：有料 1,000円(税込) /1公演/お子さま1人あたり

※キャンセルの場合は必ずご連絡をお願い致します。実施日1週間前を過ぎてのキャンセルの場合、預かり料金（1人/1,000円）が発生致しますので予めご了承ください。

This is the application form for the ROHM Theatre Kyoto 2025 childcare service. Please select the performance from the list below and enter your information.

Fee: ¥1,000 per performance and child

Please be sure to contact us if you wish to cancel. Please note that canceling within one week of the scheduled date will incur a fee.

■託児上の注意点｜Special Requests
集団保育でお過ごしになる際のお子様のご様子で気になる点、特に「アレルギーや医師からの診断事項等留意点、集団保育でお過ごしになる際のお子様のご様子で気になる点」については、託児を行う上で重要となりますので、必ず事前にお知らせください。
Please be sure to let us know in advance of any concerns you have about your child, especially regarding allergies, doctor’s diagnoses, and behavior during group childcare.
